The Open Access Clinician recognizes the importance of individuals having timely access to services. This position allows individuals who are seeking behavioral health services the opportunity to engage in services through a walk-in clinic model. This position will be primarily be focused on adult and child initial assessments but will also provide specialized assessments both in person and through telehealth. Open Access clinicians are imbedded in the Recovery Education Centers and they provide assessment, crisis intervention, and brief individual support services to individuals with a broad range of mental health and substance abuse issues.

Performs duties independently, including polishing and other duties, according to certification authorization. The School Based Health Team Leader will assist the SBH Clinical Director in ensuring high quality services are delivered across multiple sites by providing clinical supervision, completing chart audits, providing training as needed and assisting clinicians in practice management techniques. Direct clinical services include individual, family and group therapy within the school-based health model. Responsibilities include: - Provide clinical and administrative oversite support, as determined by the SBH Clinical Director, to a team of behavioral health staff. - Maintain an active caseload at 1-2 SBH Sites (60% allocation to direct services at 80% productivity) - Assure compliance with Blue Ridge Health’s service and documentation standards as well as clinical standards through regular completion of chart audits. - Follow and support established policies, procedures and protocols related to the delivery of clinical services and maintain responsibility for the team in providing practical interpretation of the established policies and procedures. - Demonstrate knowledge of Evidenced Based Practices and effectively model implementation of such practices. - Meet consistently with team therapists and staff for the purpose of collaboration, training, supervision, connection, and support. - Monthly site visits at each assigned clinic to meet with practice manager, BH coordinator, school staff and clinicians to discuss services, referrals and address any challenges. - Build and monitor Counselor schedules in Athena to best serve clinical needs and service delivery expectations. - Serve as back up consultation to other team leaders and clinical supervisor. - Attend regularly scheduled SBH meetings, and establish individual and team meetings as needed for assigned staff - Manage new employee orientation for Counselors and support in interviewing and hiring. - Provide clinical Supervision to provisionally licensed staff and other staff as needed. - Identify individual clinic needs and priorities and bring forward to the SBH Clinical Director or Practice Manager.

The Care Coordinator is responsible for aiding the care management team by working collaboratively to support patient care plans and needs. Serving as an advocate for patients, the Care Coordinator will be part of the care management team and work in tandem with care managers, healthcare providers and community-based organizations to improve outcomes for patients they serve. These responsabilities inclulde: - Engages with patients to identify and address barriers that impede health outcomes - Implements and support Care Management interventions per the patient’s care plan or assessed community needs - Processes referrals from members of the multidisciplinary team (social work, behavioral health, community resource coordinators, pharmacy, pharmacy technician, care managers) appropriately, accurately, and timely according to established workflows - Schedules practice encounters with patients via phone or correspondence based on referrals and according to policies and procedures. - Schedules/verifies appropriate medical appointments for patients as needed. - Coordinates referrals to outside agencies as directed by interdisciplinary team in a timely fashion. - Provides education to patient/family/caregiver within scope of practice. - Serves as a liaison among the patient/family, community services, primary providers, prepaid health plans, specialists, and other care team members to coordinate services. - Maintains appropriate documentation in the Care Management documentation platform, in accordance with organizational policies and procedures.

The Pharmacy Delivery Technician coordinates and delivers prescriptions to patients of Blue Ridge Health pharmacy. Responsibilities include: - Deliver prescriptions to patients of Blue Ridge Health (BRH). - Coordinate and schedule delivery with patients. - Assist with the completion of delivery enrollment paperwork. - Deliver prescriptions to patients. - Complete and ensure receipt of payment for all delivered prescriptions in the pharmacy software system.

This position is responsible for providing direct care coordination ensuring the care delivered is effective, efficient and achieves desired patient outcomes; This position will work closely with the clinical staff in the clinic to implement coordination of patient care for their behavioral health needs. The Registered Nurse is responsible for providing clinical nursing care to clients according to established professional nursing practice standards of the American Nursing Association and in accordance with the North Carolina Nurse Practice Act and other regulatory standards. Responsibilities include: - Provide care coordination services for patients in the medical and behavioral health setting and the Medication Assisted Treatment program - Provides medication injections as needed - Understands and follow established policy and procedure under minimal supervision as well as make decisions within established clinical nursing standards and protocols - Manages multiple priorities by listening, assessing, speaking and transcribing patient information in computer - Facilitates referrals to primary care providers, specialists, healthcare facilities, and community resources and communication with providers and healthcare organizations as appropriate. - Demonstrates problem solving skills, utilizes protocols and initiates solutions to facilitate the telephone encounter to an appropriate and timely disposition - Acts as a liaison between providers, care coordinators, hospital, urgent care facility, and other health care providers - Provides patient-centered care and education focused on individual and family engagement and behavioral change, treatment compliance and health risk factor reduction.

A clinician in the Recovery Education Center  provides assessment, crisis intervention, individual and group treatment, rehabilitation, and support services to individuals with a broad range of mental health and substance abuse issues and assists in coordinating care for individuals across programs.

The Pediatrician is a member of the interdisciplinary health team of Blue Ridge Community Health Services and works in collaboration with the Medical Director, and the physician team, including family medicine residents at Pardee Hospital. The pediatrician will provide primarily inpatient pediatric coverage as well as coverage for newborns at c-section deliveries. Other duties may include emergency department consultation and outpatient pediatric care as well as supervision of advanced practitioners.

The Registered Dietitian is a member of the interdisciplinary health team and provides medical nutrition services and nutrition counseling in a primary care practice setting. Responsibilities include: - Evaluate nutritional status of individuals and provide nutrition counseling and consulting to individuals, as well as collaborating with health professionals within the organization. - Study and analyze current scientific nutritional studies to assist in developing standards, guides for use in Blue Ridge Community Health Services.  - Plan, evaluate and conduct nutrition education programs and develop educational materials for various audiences and at all BRCHS sites. - Responsible for supporting organizational mission and goal achievement, organization quality goals, and providing safe, patient-center care.

The Data Entry Clerk will work to ensure administrative and medical data is recorded and maintained accurately for use throughout organization. The Maintenance Technician performs a variety of maintenance and repair duties for facility locations and on facility equipment including, but not limited to, in the areas of carpentry, mechanical, electrical, pneumatic, hydraulic, and HVAC systems troubleshooting and repair. Responsibilities include: - Diagnose problems, replace or repair parts, test, and make adjustments to equipment and clinic facilities - Performs regular maintenance activities per BRH maintenance requests submitted by management and as directed by the Facilities Manager. - Performs critical/urgent maintenance activities for emergency situations as directed by the Facilities Manager. - Read and interpret equipment manuals and work orders to perform required maintenance and service. - Take any equipment out of service that cannot be repaired and notify site manager(s) of any issues related to equipment that cannot be repaireda. Assists in the completion of routine monthly maintenance review of BRH locations and equipment for any needed preventive maintenance issues. - Performs regular preventive maintenance activities on machines, equipment, and BRH facilities. - Detect faulty operations, defective materials, and any other issues, reporting those and any other unusual situations to the Facilities Manager. - Comply with all OSHA, federal and state safety regulations and maintain clean and orderly work area(s).

The Site Coordinator is responsible for managing patient care schedules, delivering excellent customer service, and providing support staff supervision to ensure the highest degree of efficiency in patient care and patient flow at BRH clinic locations. Responsibilities include: - Assists in the completion of routine clinic tasks. Ensures consistency in maintaining: stock supplies, forms, sterile instrument inventory, equipment, appropriate exam room inventory and set-up, appropriate disposal of biohazard material, etc. Timing and assignment of tasks determined by - Practice Management Staff at clinic location. - Follows protocols and performs accurate and thorough work-ups and check-ins of all patients. - Assists with procedures with the providers including setting up treatment rooms, patient education, and completion of appropriate paperwork - Appropriately documents patient information in patient record-including but not limited to medication reconciliation and updated health histories. - Works assigned schedule, including night clinic or weekend hours, per clinic site hours of operation. - Participates in pre-visit planning including structured communication with care team. - Reviews charts prior to appointments and retrieves any needed information from physicians, pharmacies, or other agencies to complete the patient record - Assists with pharmacy refill requests, patient call backs, and other patient care related paperwork as directed by providers or nursing staff. - Completes appropriate and timely documentation in the electronic medical record and any other applicable forms/tracking logs, etc. - Maintains maintenance and control logs on medical and storage equipment. - Ensures that referrals to outside specialty providers, radiology, labs, and patient hospitalizations are tracked and follow-up is completed in a timely manner.

Responsibilities include: - Facilitate group and lead divided groups when the number reaches 12 participants - Facilitate morning and evening groups including groups that occur on holidays - Ensure there is gender specific information and offerings under the supervision of the SAIOP team leader - Support intake requirements such as the SAIOP checklist, NCTOPPS, orientations to SAIOP, person centered planning, and crisis/enhanced crisis planning - Provide case management activities including transportation, connecting with other recovery programs internal and external, connecting with higher levels of care, connecting with primary care and specialists, connecting to employment support, connecting with parenting support and connecting to social activities and social supports - Participate in graduations and celebrating of milestones - Provide skill building during individual sessions around relapse prevention, recovery planning, managing triggers and cravings, harm reduction, self care, and parenting skills. - Provide outreach and trainings about the SAIOP program as well as on basic recovery skills such as stage of change and harm reduction to the community with the guide of the SAIOP team leader - Participate in contacting those on the extra support list and following up with SAIOP participants who miss SAIOP, need extra support or are stepping down from services - Help to plan and organize group curriculum. - Support in drug screenings and coordination with the office of probation.  

The Pediatric Triage Nurse is responsible for providing clinical assessment and triage to pediatric patients by telephone aided by decision support protocols, established clinical nursing practice standards subscribed to by American Nursing Association and in accordance with the North Carolina Nurse Practice Act and other regulatory standards. Responsibilities include:  - Performs comprehensive telephone assessment to primarily pediatric patients by utilizing symptom-based protocols for all telephone encounters that require triage or advice. - Understands and follow established policy and procedure under minimal supervision as well as make decisions within established clinical nursing standards and protocols. - Manages multiple priorities by listening, assessing, speaking and transcribing patient information in computer. - Facilitates referrals to primary care providers, specialists, healthcare facilities, and community resources and communication with providers and healthcare organizations as appropriate. - Demonstrates problem solving skills, utilizes protocols and initiates solutions to facilitate the telephone encounter to an appropriate and timely disposition. - Acts as a liaison between providers, care coordinators, hospital, urgent care facility, and other health care providers.

The Qualified Professional works closely with the primary behavioral health clinician, and is responsible for; developing, implementing, evaluating and revising Person-Centered Plans which are reflective of each individual’s strengths, needs and preferences, providing symptom education to each individual to identify their unique symptoms and understand their diagnosis, providing supportive counseling (i.e. problem solving, role-playing, modeling and support), social-skill development, and assertiveness training to increase each individual’s social and interpersonal activities in community settings, providing crisis intervention and support in a manner consistent with each individual’s Crisis Plan, and collaborating and coordinating with various persons and organizations including schools, courts, probation officers, physicians and other healthcare providers, hospitals, law enforcement officers, social services, family members and community agencies.

Care Manager addresses the needs of the population served by assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required and using communication and available resources to promote quality, cost-effective health outcomes. Ensures the coordination and continuity of health care for patients as they transition from one facility to other settings. These responsibilities include: - Oversight of Care Management services and activities based on care management standards of practice for enrolled populations. Work with patients/caregivers, to identify and address behavioral, social, cultural, and environmental strengths and barriers as it relates to his/her diagnosis, treatment, and access to care.  - Implement Care Management interventions, set goals, and develop the plan of care based on transitional care discharge plans/instructions, the comprehensive assessment, and patients’ goals.  - Implement patient-centered plans using therapeutic skills and techniques such as trauma-informed care, motivational interviewing, strengths-based, and solution-focused modalities.  - Facilitate and provide education to patient/family about clinical diagnosis, medications, available resources, prevention, and risk factors to achieve optimal self-management,  - Monitor quality and effectiveness of interventions to the enrolled populations by setting patient-centered SMART goals in collaboration with the patients’ and families’ identified goals.  - Delegates tasks and referrals to members of the care management team appropriately, accurately, and timely according to established workflows.  - Serve as an advocate and liaison among the patient/family, community services, primary providers, prepaid health plans, specialists, and other care team members to coordinate services.  - Work collaboratively with multi-disciplinary team members to facilitate achievement of desired health outcomes.

As a Patient Access Representative, you will work as an integral member of the team by providing excellent customer service to all internal and external customers of BRH.  This role includes assisting patients with check-in and check-out during appoints while making sure all patient data is up to date and remains confidential. Other responsibilities include answering patient phone calls to assist with scheduling appointments.

The MAT Project Director (PD) will provide oversight to the MAT consortium and RCORP grant and DHT MAT grant.  This position will also provide direct MAT services. Other responsibilities include: - Provide oversight including fiscal and personnel management, community relations, advocacy for implementation of activities, strategic planning, technical assistance, and evaluation.  - Present the RCORP consortium members with regular status reports - Coordinating with the Data Coordinator to meet HRSA and other reporting requirements. - Direct provision of MAT services

The Referral Coordinator is an integral member of the clinical team of BRH and works in cooperation with the medical providers to schedule referral and specialty appointments for our patients. The Referral Coordinator assumes the responsibility for electronic medical records maintenance according to the departmental guidelines and ensures follow-up for referral appointments through communication with medical providers and BRCHS patients.

The Psychiatrist is responsible for evaluating, diagnosing and treating patients with behavioral health and substance use disorders. Responsibilities include: - Position is to provide ACTT psychiatric services and oversight for our team that serves Cherokee, Clay and Graham counties. - Analyze and evaluate patient data and tests or examination findings to diagnose nature and extent of behavioral health disorder. - Prescribe, direct, and administer psychotherapeutic treatments or medications to treat behavioral health, emotional, or behavioral disorders. - Examine or conduct laboratory or diagnostic tests on patient to provide information on general physical condition and behavioral health disorder. - Advise and inform guardians, relatives, and significant others of patients' conditions and treatment. - Design individualized care plans, using a variety of treatments. - Collaborate with other behavioral health professionals to discuss individual patient treatment plans and progress.  - Maintain patient information and records, including social and medical history obtained from patients, relatives and other professionals. - Counsel outpatient appointments during office visits. - Serve on committees to promote and maintain community behavioral health services and delivery systems. - Act as a Collaborating Provider for Psychiatric Mental Health Nurse Practitioners. - Responsible for understanding and following the BRCHS-Meridian Performance Improvement Plan and established safety policies and procedures. - Responsible for understanding and participating in all aspects of the BRCHS-Meridian Corporate Compliance Program. - Builds a positive work environment by working cooperatively within department and with other areas through teamwork, interpersonal skills, and personal improvement/initiative.
