Blue Ridge Health is currently seeking a Behavioral Health Care Manager to be part of our Collaborative Care team in Hendersonville, NC.
The Behavioral Health Care Manager (BHCM) is a core member of the Collaborative Care (CoCM) team and is responsible for coordinating and delivering evidence-based, measurement-based behavioral health services within primary care. The BHCM partners closely with primary care providers, psychiatric consultants, and interdisciplinary teams to support patients with common mental health and substance use disorders.
This role emphasizes population-based care, systematic follow-up, registry-driven tracking, and brief interventions aligned with Collaborative Care Model standards. Responsibilities include:
