What You'll Do:

The Lead Patient Navigator is a member of the clinic interdisciplinary team responsible for leading the Patient Navigator team to improve the outcomes of the team. Responsibilities include:

  • Serve as a knowledge base and act as a resource for Patient Navigators
  • Train all new Patient Navigators, obtain access to all portals needed to complete Patient Navigator job duties
  • Provide up to date information to all Patient Navigators. Communicating changes in processes for Charity Care, Open Enrollment, Medicaid, etc. as it is released.
  • Assist during open enrollment periods to ensure Patient Navigators complete training on time and have knowledge needed to assist patients during this period
  • Oversee the auditing process for discount applications to ensure all audits are being completed accurately and in a timely manner.
  • Identifying trends on slide audit errors and provide feedback to office managers to improve errors
  • Oversee tracking of assistance (food, housing, Insurance, etc.) provided to patients. Ensure Patient Navigators are actively logging the assistance they are providing. Provide data to management team as requested.
  • Assess any needs based on reporting measures and communicate with supervisor

What We're Looking For:

  • High School Diploma or equivalent required.
  • Community Health Worker Certification required or obtained within first year of employment.
  • Bachelor’s degree in healthcare or social services related field, preferred. Equivalent combination of education and experience is acceptable.
  • Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private health insurance, SNAP, and other local, state, or national social safety net programs strongly preferred.
  • Ability to be certified as a Certified Application Counselor (CAC) and Senior’s Health Insurance Information Program (SHIIP) counselor within one month of hire.
  • Valid driver’s license and reliable transportation, required.
