What You'll Do:

This individual functions as a clinical member of our multidisciplinary behavioral health team and provides treatment, rehabilitation and support services. Responsibilities include:

  • Serves as the primary point of contact for referrals, appointment scheduling, and linkage to appropriate levels of care.
  • Screens and enrolls patients in the Opioid-Based Outpatient Treatment (OBOT) Program.
  • Leads the client intake process, including screening, education/review of program expectations, and completion of enrollment paperwork.
  • Provides complex care coordination across multiple life domains and disciplines, including communication with community partners, referrals to outside agencies, and management of other care needs.
  • Coordinates care with our regular Medication for Opioid Use Disorder (MOUD) services to ensure seamless patient support.
  • Uses motivational interviewing and shared decision-making to address barriers and promote full participation in treatment.
  • Attends warm handoffs when patients in the clinic request contact with the OBOT program.
  • Acts as the primary source for supporting the OBOT team in patient tracking.
  • Ensures effective collaboration and communication between members of the healthcare team to support care coordination activities and program goals.
  • Consults with medical and behavioral health providers and other team members to resolve client care issues.

What We're Looking For:

  • Highschool Diploma or equivalent required
  • Professional Degree in Human Services preferred
  • CADC certification or similar credential/experience in substance use treatment
  • Certified Medial Assistant required
  • Excellent listening and communication skills
  • Bilingual preferred
  • Experience with EHRs
