What You'll Do:

The Community Health Worker (CHW) Coordinator is a front-line public health worker that works as a liaison between BRH CHW’s and the Vulnerable Populations Manager. The CHW Coordinator assists the BRH CHW’s in achieving their goals of helping to increase access to health care and community resources in the communities they serve so as to improve health outcomes. Responsibilities include:

  • Conduct street outreach and fieldwork in the community
    Connect individuals to healthcare and social services
  • Guides CHWs in scheduling, planning, and executing community events and health screenings
  • Assists CHWs in preparing for events, including:
    i. Health education sessions
    ii. Health screenings
    iii. Special outreach initiatives
  • Develops and/or maintains up-to-date educational materials for community distribution
  • Strengthen communication between clinic sites and CHWs
  • Develop CHW skills and competencies to effectively fulfill their roles
  • Upholds the organization’s mission and objectives, including:
    i. Staying informed on community health initiatives
    ii. Serving as an organizational representative in internal and external settings
    iii. Participating in required trainings and professional development opportunities

What We're Looking For:

  • High school diploma or equivalent required
  • Medical Assistant licensure or prior experience preferred
  • Bachelor’s degree in Public Health, Business Administration, Communications, or a related field strongly preferred (equivalent experience considered)
  • Strong organizational skills and the ability to work autonomously.
  • Effectively manage assignments and projects.
  • Ability to prioritize and respond to multiple requests within requested time frames.
  • A valid driver’s license is required.
  • Strong interest in working with an interdisciplinary, non-profit healthcare team as well as culturally and socioeconomically diverse community and patient population.
  • Proficiency with Microsoft Office programs and Electronic Medical Records.
  • CHW Certification
              If no CHW Certification, must obtain certification within 18 months of hire date
