Blue Ridge Health is currently seeking a Clinical Outreach Specialist to be part of our Vulnerable Populations Team.
The Clinical Outreach Specialist is a key member of our interdisciplinary health team, serving as a clinical community health worker. In this role, they actively engage with communities affected by substance use, conducting vital health screenings such as HIV, Hepatitis C, and Blood Glucose in the field. Additionally, they provide essential health education and facilitate enabling services like transportation, patient referrals, and follow-ups to both in-clinic services and external community resources. The Specialist also offers limited case management to clients in our harm reduction program and supports outreach efforts for other vulnerable populations as needed by the department.
