What You'll Do:

The Peer Extender plays a unique and vital role in supporting one’s journey of recovery. They serve on an interdisciplinary tailored care management team whose focus is to deliver integrated, whole person care addressing a member’s physical health, behavioral health, long-term services and health needs and/or pharmacy needs. Responsibilities include: 

  • Performing general outreach, engagement, and follow-up with members
  • Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation)
  • Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual)
  • Sharing information with the care manager and other members of the care team on the member’s circumstances
  • Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services
  • Participating in case conferences
  • Support the care manager in assessing and addressing unmet health-related resource needs

What We're Looking For:

  • High school diploma or equivalent required
  • Certified Peer Support Specialist with lived behavioral health experience
  • Drivers License
  • Previous Peer Support Specialist (PSS), Community Health Worker (CHW), Community Navigators or Family Navigators a plus
